MEDICAL NEEDS

Please complete any forms associated with your campers medical needs, if applicable.

Forms will be submitted electronically prior to your campers first day of camp. Please note some forms require a physician's signature.

Medications must be in their original container labeled with the child’s full name and date brought to camp.
Medication will be dispensed per doctor’s instructions or according to the label.
Expired medication will not be administered.
