Please complete any forms associated with your campers medical needs, if applicable.
Forms will be submitted electronically HERE prior to your campers first day of camp. Please note some forms require a physician's signature.

Medications must be in their original container labeled with the child’s full name and date brought to camp.
Medication will be dispensed per doctor’s instructions or according to the label.
Expired medication will not be administered.
If any of the following conditions apply to your camper AND will require camp staff to administer medication, you must also complete the "Medication Authorization" linked above.
